U.S. Physician CustomVue Certification Course

Course Content
The Physician CustomVue Certification Course and Course Review contains important information on performing CustomVue treatments for myopia and myopic astigmatism with the VISX STAR S4 Excimer Laser System and WaveScan WaveFront System.

Your Learning Experience
Plan approximately 2 hours and 15 minutes to complete the Physician CustomVue Certification Course and Course Review. When you have completed the course and course review, a Certificate of Completion will be presented for you to print. Please also review the Professional Use Information Manual for the VISX STAR S4 Excimer Laser System and WaveScan WaveFront System (CustomVue Treatments).

Log-In Instructions to the VISX eLearning Portal
This course is designed for IBM®-compatible computers with Internet Explorer 5.5 and higher, Netscape Navigator® 7.0, and with Windows® 98 or higher. The course is not compatible with Apple Macintosh® computers. To login to the VISX eLearning portal and gain access to the Physician CustomVue Certification Course:

1. Click the "Login to LMS (Learning Management System)" button.
2. At "Login ID", type in your username as a combination of your first initial and last name as one word (i.e., John Smith would be jsmith). Use lowercase letters only.
3. At "Password", your initial password is visx in lowercase letters only.
4. Login instructions and a help pop-up box is available on this page to provide you with guidelines for logging in.
5. Click the "Enter" button.
6. Find the Course Title "Physician CustomVue Certification Course, " click it and follow the onscreen directions to proceed within the course.

To Begin the Physician CustomVue Certification Course Review
To begin the Physician CustomVue Certification Course Review, you will be asked to enter some information that would be easiest for you to have gathered before you proceed to the Course Review.

1.   

If you use the laser installed at your location, you will need the laser serial number.
  There are four ways to locate the serial number of the laser:

  • There is a small silver label on the back of the laser with the serial number.
  • The serial number of the laser is on a VISX invoice for vision key cards.
  • The serial number is on the VisionKey card.
  • The serial number is on the operative report.
2.    If you use an open access center laser, you will need the main phone number of the center.
3.    If you use a mobile laser, you will select from a list on the screen.
Your information provided is intended to be used to provide you with certification and clinical education update information only.

Reprinting Your Certificate of Completion
You can reprint your Certificate of Completion at any time after you have completed the Physician CustomVue Certification Course. Simply login to the course again, and at the bottom of the Menu screen click the Print Certificate button.
