The following link allows you to take the VISX On-line assessment and print your Operator's Certificate. Your log-in name will tell the system whether you are taking the Laser assessment or the WaveScan assessment. You MUST have completed an on-site training course with the Refractive Specialist. You will not be able to log-in unless the Refractive Specialist has submitted your contact information.
